HTA Conversations for Change Consumer Symposium 2022 feedback survey

Overview

Thankyou for your participation in the Health Technology Assessment (HTA) Consumer "Conversations for Change" 2022 Symposium.

We would be grateful if you could let us know via the online survey whether or not this was a useful event, and provide any suggestions for improvement. 

• The survey may take 2-15 minutes to answer, depending on how many questions are answered or comments provided.
• All but two questions are optional.
• You can provide your feedback anonymously.                                        There is the option to provide your name, in case you have questions or feedback you might like us to respond to personally. You can also provide your email address if you would like a copy of your responses (this email will not be saved with your responses so you can still remain anonymous).

By submitting the survey, you consent to us holding and using the information submitted for the purposes of understanding and reporting on 2022 Symposium attendee experiences and suggestions, in accordance with the Department of Health and Aged Care's Privacy Policy.

What happens next

An evaluation of the Symposium will be conducted and reported in due course, and made publicly available on the OHTA Consultation Hub.
